Few moments cause more unnecessary anxiety in a spine clinic than a patient reading their own MRI report before their appointment. Words such as "degeneration," "disc bulge," "herniation," and "stenosis" can sound alarming. But an MRI is a picture of the spine — it is not a diagnosis by itself.
The important question is not simply "What does the MRI show?" It is: "Does what we see on the MRI actually explain the patient's symptoms?"
Understanding this distinction can completely change how we interpret a spine MRI.
Disc Degeneration: A Finding of Aging, Not Necessarily Disease
"Disc degeneration" describes structural changes that commonly occur in the discs as we age, including loss of water content and, over time, loss of disc height. These changes are extremely common, even in people who have no back pain at all.
Think of it as similar to changes that occur elsewhere in the body with age. The presence of some degenerative change does not automatically mean that something is wrong or that it needs to be treated.
An abnormal-looking disc on an MRI does not necessarily mean that the disc is the source of your pain.
Disc Bulge vs. Herniation: Terms That Are Often Confused
A disc bulge is a broad extension of disc tissue beyond its usual boundary. A disc herniation is a more focal displacement of disc material. Depending on its size and location, it may come into contact with or compress a nearby nerve.
But these terms primarily describe what the disc looks like. They do not, by themselves, tell us how severe the patient's symptoms will be.
A large disc bulge can cause no symptoms at all. Conversely, a relatively small disc herniation in the right location can cause severe leg pain if it affects a nerve root. Location and clinical correlation often matter more than size alone.
Spinal Stenosis: Narrowing Does Not Always Mean Disease
Spinal stenosis means narrowing of the space available for the spinal cord or nerve roots. It can occur in the central spinal canal, the lateral recesses, or the neural foramina where the nerve roots exit the spine.
Some degree of narrowing becomes increasingly common with age, and it does not always cause symptoms. The finding becomes clinically important when the anatomical narrowing corresponds with the patient's symptoms and examination.
For example, a patient may have significant foraminal narrowing at L4–L5 on MRI but no symptoms related to the L4 nerve root. In that situation, the MRI finding may not be the cause of the patient's problem.
Incidental Findings: The Most Important Concept
This is one of the most important things to understand about spine imaging: Abnormal findings on an MRI are common even in people who feel completely well.
Large studies of asymptomatic people have demonstrated that disc degeneration, disc bulging and other degenerative findings become increasingly common with age. In other words, an abnormal MRI does not necessarily mean an abnormal patient.
This is why reading an MRI report without understanding the clinical context can create unnecessary fear. A report may contain several abnormalities — but only one, or sometimes none, may actually be responsible for the patient's symptoms.
The Correlation That Actually Matters
An MRI becomes much more meaningful when it is interpreted alongside:
- The patient's symptoms — where the pain is, whether it radiates, numbness or weakness, and how the symptoms behave
- The physical examination — strength, reflexes, sensation, gait and other relevant findings
- The clinical history — when symptoms started, how they have progressed and what makes them better or worse
- The imaging — MRI, X-rays or CT when appropriate
Consider a patient with an L4–L5 disc herniation compressing the L5 nerve root who has typical L5-pattern leg pain, corresponding sensory changes and weakness. In that situation, the MRI finding has a strong clinical correlation.
Now imagine exactly the same MRI finding in someone with only occasional, non-specific lower back pain and no leg symptoms. The same image may have very little clinical significance.
The image has not changed. The patient has. And that changes how the image should be interpreted.
The Bottom Line
An MRI is one of the most powerful tools in modern spine care. But an MRI should never be interpreted in isolation.
A scan can show degeneration, bulging discs, herniations and narrowing — sometimes several of them at once. The challenge is determining which findings matter and which are simply part of the normal spectrum of spinal changes.
The real skill in spine care is not simply identifying an abnormality on an MRI. It is understanding whether that abnormality explains the patient's symptoms — and whether treating it is likely to make a meaningful difference.
Don't treat the MRI. Treat the patient.
